What does a pre workout actually do?

Is it just caffeine?
Mostly, in practice. A review of multi ingredient pre workout supplements found that an acute dose can modestly improve muscular endurance and mood, with mixed results for force and power, and that chronic use alongside training may add a little lean mass, but that formulations vary enormously and many hide their ingredient amounts inside proprietary blends. The most direct test is telling: in a crossover study, bench press repetition volume was greater after a plain dose of caffeine than after a multi ingredient supplement that contained the same caffeine plus beta alanine, citrulline malate, arginine, taurine and tyrosine. The authors concluded the blend was less ergogenic and less cost effective than caffeine alone. It is a small study, but it points the same way as the ingredient evidence.
What about the other ingredients?
The citrulline and beta-alanine reviews address different participants, protocols and outcomes. A strength result does not answer every endurance question, and repeated supplementation does not establish an immediate effect from one serving. A large enough trial showing a specific multi ingredient formula beats an equal caffeine dose for strength or hypertrophy outcomes.
